Madison Jade Ballard

Canfield - November 27, 2005 - Madison Jade Ballard, daughter of Benjamin R. and Tiffany N. Medvec Ballard, was born still on Sunday evening, November 27, 2005 at St. Elizabeth Medical Center, Youngstown.

Madison is survived by her parents, of Canfield, maternal grandparents, John W. and Suzanne Medvec of Canfield; maternal grandfather, Charles W. Custer of Columbiana; paternal grandmother, Maria King of New Waterford; maternal great grandparents, Robert and Vera Bortmas of Washingtonville; and a large and supportive family.

She was preceded in death by her paternal grandfather, Terry Ballard, an uncle, Terry L. Ballard, and a great uncle, Randy Bortmas.

Services will be held at 1:30 pm Saturday, Dec. 3, 2005 on at the Woods-Rettig Funeral Home, Leetonia with the Reverend Dan Cipar of Our Lady of Lourdes Catholic Church, East Palestine, officiating. Burial will follow in Oakdale Cemetery, Leetonia. Friends may call at the funeral home one hour prior to the services on Saturday.
